Section 04 — Informed Consent & Assumption of Risk

I, the undersigned Client, voluntarily consent to participate in fitness assessments, exercise testing, personal training, and/or group training sessions, and wellness regimens provided by The Forge of Keller LLC and its staff.

Assumption of Risk I acknowledge and agree that the sole risk of injury or harm resulting in any manner from my choosing to participate in such regimens, programs, and services rests entirely with me to the extent that I do not disclose my health conditions, medications, or drug use in advance.

Section 05 — Waiver of Liability

Release & Waiver By signing below, I acknowledge and voluntarily assume all risks associated with participation in training activities. I hereby release, waive, indemnify, and hold harmless The Forge of Keller LLC, its owners, trainers, employees, contractors, and affiliates from any and all claims, demands, damages, or causes of action arising from injury, loss, or death related to participation, including claims of negligence to the fullest extent permitted by law.

Section 06 — Cancellation Policy

Membership

30-day written email notice required before next draft date
Permitted only after initial 4- or 12-month term is fulfilled
No refunds once a membership draft has processed

Group Classes

Minimum 6-hour notice required to cancel
Failure to cancel or attend results in forfeiture of that class
Unlimited memberships may incur a $10 late cancel / no-show fee

Personal Training

Minimum 24-hour notice required to cancel or reschedule
Sessions cancelled with proper notice may be rescheduled within same month
No-shows or late cancellations result in forfeiture of the session
Group training sessions are not eligible for make-ups

Section 07 — Independent Medical Provider Disclosure

Wellness on Demand · Independent Third-Party Provider

The IV therapy and wellness services referenced on this website are offered and performed exclusively by Wellness On Demand, an independent third-party medical provider operating under its own professional licenses, medical supervision, and insurance coverage.

The Forge of Keller is not a medical provider and does not supervise, direct, or control the provision of medical services. Any scheduling assistance provided through The Forge of Keller is for convenience only.

All payments for medical services are processed by Wellness On Demand. All medical waivers, informed consent forms, and liability releases are executed directly with Wellness On Demand and are maintained exclusively by that provider. All questions regarding medical treatment, eligibility, risks, and outcomes should be directed to Wellness On Demand.
